All appointments must be requested at least one full week prior to appointment, unless case has been approved. A release of information and waiver of non-responsibility must be signed, prior to appointment. All no-shows, last minute cancellations w/o valid reasons, and any aggressive-like behaviors will not be eligible for additional services following the appointment for 30 days or permanent, depending on severity of the situation. A police report may also be deemed appropriate for threatening behaviors.

Legal Appointments: We offer transportation for clients who do not have income of any kind to the following types of legal appointments.

• Subpoenaed court hearings-documentation must be provided, current, & valid.

• Jury duty-documentation must be provided, current, & valid.

• Supervised visit-Family court that is court ordered-documentation must be provided, current, & valid.

• Drug Court that is court ordered. documentation must be provided & valid.

Medical Appointments: We offer transportation for client(s) who do not covered by current healthcare, insurance, Medicaid/Medicare, and/or any other health coverage plans that offer transportation via health coverage plan and do not qualify for healthcare, insurance, Medicaid/Medicaid/Medicare, and/or any other health coverage plans that offer transportation via health coverage plan.

• Physical therapy, Mental Health, and pregnancy related appointments- documentation must be provided, current, & valid.

DISCLAIMER: Any Emergency appointment will be encouraged & referred to calling 911, as we, Share your Testimony, will NOT held liable for any and all kinds of life threatening or non life threatening injuries that occur or may occur.

DISCLOSURE: Share your Testimony staff cannot assist with physically picking up clients or assisting in the carrying of clients who are not able to do for themselves, as this is a liability that we are not trained for or licensed to do so. We will do our best to provide additional resources and/or referrals to another entity. The vehicle provided is also not wheelchair accessible.
